Skilled Nursing Services
Our team of skilled nurses works closely with the patient's physician to develop an individualized plan of care for each patient. Our nurses provide compassionate care in the patient's home. They assess and monitor the patient's healthcare needs, provide education to the patients and their caregivers, and work closely with the patient's physician to promote continuity of care. Skilled nursing services include all of the following:
  • Patient assessment and coordination of care
  • Disease state management teaching
  • Infusion Therapy- IV medications, TPN
  • Diabetic education
  • Urinary catheter insertion and maintenance
  • New medication teaching
  • Injections
  • Wound Care
  • Pain management
  • Dressing changes
  • Ostomy care
Wound Care
Our wound care consultant assists our nursing staff with the evaluation and treatment of all kinds of wounds. She provides consultation and education for the patient and the patient's primary caregiver. The wound care consultant helps the nursing staff with the treatment of the following kinds of wounds:
  • Pressure ulcers
  • Vascular ulcers
  • Draining wounds
  • Stomas and fistulas
  • Open injury wounds
  • Amputations
Physical Therapy
Our Physical Therapist are highly educated and trained to deliver extensive therapy services in your home. Whether you have had a joint replacement, a stroke, or another orthopedic injury, our staff of caring professionals can help you recover quickly right in the comfort of your own home. Our therapist will work with your physician to create an individualized therapy plan just for you. Our physical therapy services include, but are not limited to:
  • Gait training
  • Balance training
  • Strengthening exercises
  • Weight bearing exercises
  • Post-operative rehabilitation
  • Post CVA rehabilitation
  • Recommendation and training in assistive device options and use
Speech Therapy
Our Speech Therapists work with patients who have speech and language impairments, and swallowing problems. Many of these patient's have either had a stroke, traumatic brain injury, head or neck cancer, Parkinson's disease, amyotrophic lateral sclerosis (ALS), multiple sclerosis, vocal cord nodules, or another debilitating neuromuscular disorder. Speech therapists evaluate and provide therapy for the following problems:
  • Speech and language disorders (aphasia)
  • Swallowing disorders (dysphagia)
  • Voice disorders
  • Special communication techniques
Nursing Aides
A patient may qualify for a home health aide if they require assistance with personal care and are being treated at home by a home health nurse, physical therapist, occupational therapist, and/or a speech therapist. The primary function of our home health aides is to provide assistance with personal care and limited household functions to ensure that the patient is in the safest, most healthful environment possible. Such assistance may include:
  • Help with bathing and dressing
  • Help getting in or out of bed
  • Help with mouth or hair care
  • Assistance with training in self-care techniques
  • Helping with exercises
  • Changing bed linens
  • Preparing a light breakfast or lunch
With special doctor orders a Home Health Aide may provide additional personal care services, provide laundry service, complete errands and do grocery or other shopping duties.

Occupational Therapy
It is the goal of our Occupational Therapists to help our patients regain their independence in the activities of daily living. These include bathing, dressing, meal preparation and eating. This enables the patient to remain at home rather than have to enter into an assisted living facility or a nursing facility. Our Occupation Therapists will evaluate the patient and teach techniques to improve their level of independence in their ADL's, and aggressively work with them to restore their physical function. The patient is sometimes fitted with orthotic devices and taught the use of self-help devices to help maximize there level of independence. Our occupational therapy services include, but are not limited to:
  • Home safety evaluations
  • Sensory-motor evaluation
  • ADL training
  • Design, fabrication, fitting and training of prosthetic and self-help devices
  • Teaching energy conservation skills
Medical Social Worker
Our Licensed Medical Social Workers assist our patients and their families with assistance in many areas, including but not limited to:
  • Providing in-depth assessments of the patient’s mental status
  • Providing counseling for patient’s and their families
  • Assists caregivers in coping with behaviors and stress
  • Acts as a community resource
  • Does long term planning
  • Performs crisis intervention for patient’s and their families
  • Monitors the patient’s compliance with treatment plans and medications
  • Evaluates the effectiveness of psychopharmacology
